C-Match Syracuse, NY: $149,995 to establish a Fetal and Infant Mortality/Morbidity Review/Registry (FIMMRR) for Onondaga County, providing critical feedback to healthcare providers, which can be used for Continuous Quality Improvement.
Catholic Charities of Buffalo Buffalo, NY: $150,000 to implement software and hardware system to streamline billing, records, and appointment scheduling.
Cattaraugus County Department of Aging , Olean, NY:$6,000 to expand the availability of Powerful Tools for Caregivers to benefit caregivers in Cattaraugus County. The funds will cover the training of class leaders and replacement leaders over a two-year period.
CCH Home Care and Palliative Services , Liverpool, NY: $11,308 to establish a home telehealth program. Through this grant, CCH will lease monitoring equipment for use with an estimated 50 Medicaid clientele who are long term patients with chronic conditions.
Center for Excellence in Health Care Journalism , Western & Central NY: $7,000 to provide scholarship support for WNY and CNY health reporters to participate in the Association's Urban Health Journalism Workshop, to be held in New York City on October 12 & 13, 2007.
Center for Hospice & Palliative Care Cheektowaga, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to raise the awareness of end-of-life options for adults with developmental disabilities and reduce unnecessary transfer to higher levels of care.
Center for Hospice & Palliative Care Cheektowaga, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve the transition to end-of-life care for appropriate patients in a medical practice.
Center for Hospice & Palliative Care for the Coalition for Health Care Decision-Making, Cheektowaga, NY: $86,500 to support a two year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $28,641.13.

Community Care of WNY (Home Care & Hospice)- Olean, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to increase the ability of older patients with CHF and COPD to better manage their care and reduce avoidable hospitalizations and emergency care using a transition coach.
Community Concern of WNY Derby, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve the success for frail elders discharged from a TLC facility using a transition coach.
Community Dispute Resolution Center , Ithaca, NY: $5,600 to expand the Community Dispute Resolution Center's Wise Talk Program to include a peer referral component. Wise Talk offers mediation to elders, their families and the professionals with whom they interact.
Concept Systems Ithaca, NY:  $17,650 to engage stakeholders from three neighborhoods in the City of Buffalo in a consensus building process that will identify priorities and guide the development of strategies to assist frail elders to age in place in the identified neighborhoods.
Continuing Care Foundation (an organization of the Catholic Health System), Buffalo, NY: $300,000 from the Community Health Foundation’s donor advised fund at the Community Foundation for Greater Buffalo for development of a Program of All-Inclusive Care of the Elderly (PACE) in Lackawanna, NY.
Cornell Cooperative Extension of Monroe County , Western & Central, NY: $174,829 to serve as administrative bookkeeping support for the CHFWCNY Health Leadership Fellows program.
Cornell Institute for Translational Research on Aging Ithaca, NY: $107,646 to provide an objective evaluation of the cost-effectiveness and feasibility of the Project Home pilot at Loretto in Syracuse, NY – a three year demonstration program designed to respond to the wishes of an aging population to receive care in home and community based settings.
Council on Aging of Niagara County for the Niagara Caregivers Network, Niagara Falls, NY: $69,519 to support a two year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $34,262.
Council on Aging of Niagara County Niagara Falls, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to initiative a self-management program for elders to reduce unnecessary hospitalization.
Crouse Hospital Syracuse, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to reduce avoidable emergency room visits and hospitalizations as well as improve self-management using a transition coach.
Diocese of Buffalo Buffalo, NY: $73,043 to provide training to decrease bullying and increase cultural competency in the Diocese’s 20 most impoverished schools.
Erie 1 BOCES West Seneca, NY: $200,000 for planning and implementation of the Centers for Disease Control's Coordinated School Health Model in the Buffalo Public Schools.
Everywoman Opportunity Center, Inc. , Buffalo, NY: $7,500 to provide training for WNY Nuts & Bolts grantees on use of the Self-Sufficiency Calculator and assess the impact of the training.

Healthy Community Alliance , Gowanda, NY: $15,350 for the Springville/Concord Task Force to further assess the unmet needs of seniors residing in the Town of Concord and the Village of Springville and beging a planning process to address the needs.
Hospicare & Palliative Care Service of Tompkins County Ithaca, NY: Up to $30,000 to particpate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve quality of life and self-management of symptoms for patients receiving chemotherapy treatment using a transition coach. 
Hospice Chautauqua County Mayville, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to reduce unnecessary emergency care and admission for patients with CHF and COPD and will increase appropriate transition to end-of-life care.
Hospice Foundation of WNY for St. John Hospice/Buffalo House Initiative, Buffalo, NY: $100,000 to provide community outreach and recruit and train staff for hospice work in inner-city neighborhoods in Erie County
Human Services Coalition of Tompkins County for the Long Term Care Committee of the Health Planning Council, Ithaca, NY: $70,000 to support a two-year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $35,000.
Jamestown Community College Jamestown, NY: $70,000 in scholarship funds for students enrolling in a one-time program offering for dental hygienists in the rural areas of WNY.  The program is a collaboration between Jamestown, Monroe and Jefferson Community Colleges.
Jewish Family Services Buffalo, NY: $139,489 for expanding mental health services to children.
John Snow, Inc. Research & Training Institute $99,764.50 to assess the health care safety-net in western New York including - access to adequate primary care, mental and oral health, consumer experience with primary care and baseline information technology capacity of key providers.
Just Buffalo Literary Center , Buffalo, NY: $20,000. Through contracting with writers and photographers, JBLC will take the lead in writing, editing, and producing the "scenario stories" and Facilitator's Guild for the Reaching for Excellence project.
Kaleida Health Buffalo, NY: $207,872 to develop and implement a Quality Assurance Program for school based health centers at Kaleida and Catholic Health systems and Jamestown School District.  (The Catholic Health System’s and The Resource Center, on behalf of the Jamestown School District each received $30,000 of this award the support their efforts in this project.)
Kaleida Health LTC Division Buffalo, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve transition from hospital to long-term care using a transition coach.
Loretto Syracuse, NY: $100,000 to support evaluation of a three year demonstration program designed to respond to the wishes of an aging population to receive care in home and community based settings.
Loretto Syracuse, NY: $300,000 from the Community Health Foundation’s donor advised fund at the Community Foundation for Greater Buffalo for construction of a 10 bed facility to support the housing needs of current and future frail elders enrolled in Loretto’s Program of All-Inclusive Care of the Elderly (PACE).
Massachusetts Avenue Project Buffalo, NY: $35,000 for a Mobile Market and Community Kitchen that increases access to affordable, nutritious food for Buffalo's East and West Side low income residents.
McMahon Ryan Child Advocacy Syracuse, NY: $149,997 to develop web-based tracking system of abused children in Onondaga County, hire a case manager to coordinate services for these children.
Mid-Erie Counseling & Training Services- Cheektowaga, NY: $106,000 to assess vulnerable children/families in their service area to develop an outreach program and improve cultural competency of staff to provide needed mental health services.
Moving in Congregations, Acting in Hope (MICAH) , Cortland, NY: Up to $5,000 for planning and community education activities that support Cover Cortland's Children, a community-based campaign to cover 100% of Cortland County's children with health insurance by 2010.

Niagara Hospice Lockport, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve information exchange to help patients understand end-of-life care options and to improve appropriate referrals.
Northwest Buffalo Community Health Center Buffalo, NY: $26,475 to improve operational efficiency and patient flow processes by replacing outdated and failing computers.
Onondaga County Department of Aging & Youth Syracuse, NY: $6,000 to provide training for local class leaders to provide the Powerful Tolls for Caregivers.
Onondaga-Cortland-Madison BOCES Syracuse, NY: $114,638 to hire a Wellness Coordinator to facilitate implementing the Year One goals of the Syracuse City School District's Wellness Policy adopted by the District School Board. Year Two goals were supported by a grant in the amount of $108,967.
P2 Collaborative of WNY , Western New York: $6,500 to explore the interest of QI professionals in becoming involved in a more formal learning community and to generate wider participation in the IHI Conference telecast in December 2007.
Parish Nurse Ministries of NY- Williamsville, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to determine the efficacy of parish nurses functioning in the role of transition coaches to help frail elders remain in their homes following acute illness.
Pioneer Camp & Retreat Center Angola, NY: $120,900 to train staff to better address the needs of foster children and recruit advanced social work students to better serve these children.
Planned Parenthood of Buffalo & Erie County Buffalo, NY: $50,000 to support the intended outcomes of the merger of Planned Parenthood of Buffalo and Erie County and Planned Parenthood of Niagara County - cost savings, provision of more services, more efficient use of revenue and greater long term stability/capacity and sustainability.
Polish Community Center Buffalo, NY: $49,658 to implement Kid Trax software to increase staff time to provide direct services to children.
Primary Care Development Corp. (PCDC) , New York, NY: $9,044.35 to facilitate prioritizing methods for engaging health care providers around improving the quality of the care they provide.
Reconstruction Home & Health Care (Beechtree Care Center Ithaca, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to double the number of people successfully discharged from nursing home to home using a transition coach.
Southern Tier Community Health Center Network- Olean, NY: $41,567 to improve operational efficiency and patient flow processes by replacing outdated and failing computers and servers.
Southern Tier Community Health Center Network, Olean NY: $15,000 to engage John Snow Inc., to assist in the process of completing their application for designation as a new Federally Qualified Health Care Center.
St. John Baptist Fruit Belt Community/Hospice Buffalo , Buffalo, NY: $50,000 for a collaboration between Hospice Buffalo and St. John Baptist Fruit Belt Community Development to fill the need for hospice residential services within the fruit belt of Buffalo's East Side through construction of an eight bed Hospice Buffalo House in the neighborhood.
Syracuse City School District Syracuse, NY: $150,000 for construction of a school-based health center at Delaware Elementary School.
Syracuse Community Health Center Syracuse, NY: $51,953 to implement and provide training for an automated immunization tracking system.
Syracuse University Syracuse, NY: $21,175 for faculty in the Community Health Foundation’s Health Leadership Fellows Program.
The Resource Center- Jamestown, NY: $100,000 to provide start-up funds for a school-based clinic at Jamestown High School.
Tompkins County Office for the Aging , Tompkins County, NY: $6,000 to expand the availability of Powerful Tools for Caregivers to benefit caregivers in Tompkins County. The funds will cover the training of class leaders and replacement leaders over a two-year period.
Total Aging in Place Amherst, NY: $300,000 from the Community Health Foundation’s donor advised fund at the Community Foundation for Greater Buffalo for expansion of the Program of All-Inclusive Care of the Elderly (PACE) to accommodate 80-100 new participants.
UB Foundation Services, Inc. , Western NY: up to $150,000 to provide leadership and support for the Reaching for Excellence Initiative, including infrastructure, speakers series support and development of community-driven priorities for health system progress.
United Way of Buffalo & Erie County , Buffalo, NY: $4,000 to support the evaluation by the UB Center for Children and Families of the summer recreation program of the "Closing the Gap" initiative.
