Request for Proposals to Evaluate Project Home

The Community Health Foundation is seeking an evaluator or evaluation team to evaluate Project Home. The selected evaluator or evaluation team will work in conjunction with the Loretto Project Home team and the CHF program advisor immediately upon award of the evaluation grant - anticipated in January 2006. CHF intends to award a total of $85,000 - $100,000 over three years for one evaluator or an evaluation team.
Project Home is a three-year demonstration program funded primarily by a grant through the New York State Department of Health to Loretto in Syracuse. The goal of Project Home is to provide individuals in need of long-term care the option of living at home or in an appropriate community setting, with the support of community services. The project seeks to demonstrate a potentially replicable pro-active discharge planning model. It will also seek to demonstrate what, if any, costs savings are achieved through a greater use of home and community-based care and what barriers prevent individuals from leaving institutional care.
